Online Enrollment Instructions

When you click on “Enroll / View Benefits" button above you will be directed to login page: 

• Your Login ID is your Member Number with the letters SC in front of your Member ID# (example: SC123456)
• Your Password is the last six digits of your social security number
• After logging in, you will be prompted to change your password 
• Click on the “2011 Step by Step Benefits Enrollment” link and follow each step.

You must elect or decline  each line of coverage.

• Please review and update your home address and phone number
• Please provide your personal email address
• And update your dependent and beneficiary information
  
  

Please check your Enrollment Confirmation carefully before exiting the online benefits system. If you see a problem or want to make a change, simply go back to the benefits section and modify your election. When you are finished, print your Benefits Enrollment Confirmation.

To enroll by telephone: Call 1-888-539-8163 and press option #1 to speak to a Member Service Representative. A representative will be able to take your enrollment elections and enroll you by phone. As verification, you will receive a confirmation of enrollment in the mail.
